This information may be held in paper or electronic form, within the practice, or sent to the hospital or clinic in which you are being treated. This information is required to make sure that we give you the best possible care and treatment.

You have the right to see and agree to what is being recorded and also to consent to who can access your record and for what purpose the information within it can be used.

Having your information available to others helps us to ensure that you are given the most appropriate care and treatment. This is especially important if you are taking medication and/or have allergies.

Everyone working in the NHS or on behalf of us has a legal and ethical duty to keep your information confidential and secure and to seek your consent to its uses.

Sometimes patient records can be used for purposes other than your own direct healthcare. Some examples of this are legal requirements, e.g. the reporting of deaths or where there is a risk to the safety of yourself or another person. Others are to help us to audit our services to improve the care we provide.

If you would like to discuss any of these issues further please contact the Practice Manager
